Theophylaktos 21 represented the see of Amyzon in Caria at the first session of the Second Council of Nikaia (the Seventh Ecumenical Council), on 24 September 787: Mansi XII 998 (Θεοφυλάκτου τοποτηρητοῦ Ἀμυζόνος; the Latin version, at XII 997, reads: "Theophylacto episcopo Amyzonis"). He apparently failed to attend any more sessions. Cf. Anthimos 4. Presumably a priest, deacon or monk.
