Theophylaktos 11 was bishop of Methymna (on the island of Lesbos); in 692 he attended the Quinisext Council at Constantinople, where he signed the canons: Ohme, Bischofsliste, p. 151, no. 48 (= Mansi XI 992) (Θεοφύλακτος ἐλάχιστος ἐπίσκοπος τῆς Μεθυμναίων πόλεως).
