Ioannes 45 was bishop of Pessinous in Galatia Secunda; in 681 he attended the Third Council of Constantinople (the Sixth Ecumenical Council) and was present at two of the last three sessions (sessions sixteen and eighteen, from 9 August to 16 September): Riedinger II 2. 686-823 (= Mansi XI 613-672). In the subscriptions to the statement of the faith, which condemned monotheletism, and to the Council, he is styled Ἰωάννης ἐλάχιστος ἐπίσκοπος (Riedinger II 2. 823, line 27 (= Mansi XI 672) reads: ἐλέει Θεοῦ ἐπίσκοπος) τῆς Πισινουντιέων μητροπόλεως τῆς δευτέρας τῶν Γαλατῶν ἐπαρχίας: Riedinger II 2. 782, lines 13-14, 823, lines 27-28, cf.
